MINOR / CHILD PARTICIPANT’S ACKNOWLEDGMENT:

FOR PARTICIPANT TO COMPLETE, IF BETWEEN 7 YEARS OLD – 17 YEARS OLD

I,                                                                              confirm that my parent(s) and/ or guardian(s) OR an Articus Coach or Director have taken the time to explain the above Participation Package Agreement with me, including the information about COVID-19, CIVID, Personal Injury Risks, assumption of Risks, the TriCity Policies & Procedures and the Anti-Harassment, Anti-Bullying, Anti-Discrimination and Social Media Policies of Articus and at the Studio. I confirm that I have reviewed all of the aforementioned documents, policies and procedures with an adult and I understand the rules and promise to follow these safety rules, policies, and instructions given and taught to me by Articus coaches and team members, to help keep myself and others safe while learning and having fun. I confirm that I am           years old, and can print and sign my own name in my own way to confirm that I understand.
